Non-Emergency Medical Transportation (NEMT)
Health First Colorado (Colorado’s Medicaid Program), provides free transportation to and from your doctor’s appointments or can help eligible customers receive millage reimbursement for trips they drive themselves to for travel expenses incurred for non-emergency travel to and from doctor appointments and other health-related appointments. Mountain Ride Transportation manages the NEMT program for Grand county and would be happy to assist you!. If you are currently on Medicaid and you like to find out more about this program and see if you are eligible please contact Mtn Ride Transportation!

What do I need to do?
1.Call toll free 1-844-Mtn-Ride (1-844-686-7433), or go to the Mtn Ride website- http://mtnride.org/, a minimum of 5 Business Days prior to your Medicaid medical appointment.
2.Have your appointment approved by our Medicaid Transportation Specialist and coordinate your driver or if you are diving yourself they will be able to provide you wit a prior authorization number for your trip.
3.Attend your appointment. (Be sure to take your reimbursement form with you and collect the proper information)
4.Submit the completed reimbursement form to your Medicaid Transportation Specialist at Mountain Ride.
