Medicare Part D

Medicare Part D is a subsidy that may help eligible applicants with the cost of prescription drugs costs.

Applicants need to be 65 years or older, and already receiving Medicare. Applicants cannot be enrolled in both the Medicare Savings Program and Medicare Part D.  Applicants may have a monthly income up to $1,239 for an individual or $1,660 for a couple. Your assets will need to be below $8,100 for an individual and $12,910 for a couple.

How to Apply
Pick up an application from our office, call to have one mailed to you, or print an application online. Bring the completed application for assistance and all required documents into our office. Applications cannot be processed without all required documents, which include:
  • Proof of identity (driver’s license, birth certificate, U.S. passport, military ID, etc.)
  • Proof of income (pay stubs, verification of Social Security income {SSI} benefits, pensions, etc.)
  • Proof of resources (bank statements, individual retirement account {IRA} / stocks / bonds, life insurance and burial policies)
  • Social Security number

Spanish applications are available.

Application Process Period
Medicare Part D applications will be processed within 60 days from the date our office receives your completed application and all required documents.
